For Patients

It is imperative that you bring your CURRENT insurance card to each visit.  If you have a digital card, you must bring a printed copy.
Patients who do not have their current insurance card will be processed as a self-pay patient, responsible for the charges at the time of visit.  We will provide you with an itemized receipt for the visit that you may submit to your insurance carrier.  We strive to help all our patients maximize their insurance benefits and can only do that when we have a copy of your card.

Pay My Bill

We now offer the option of quick online payment of your bill through a secure Payment Portal.

Patients also may access and pay on their account through our Patient Portal.  If you have not already registered for our portal, please use the Contact Form on the right-hand side of this page to request the invitation to set up your portal account.
